• Completed and signed application (sign last 2 pages)
• Copy of Rental lease or Mortgage payment statement
• Pay stubs for LAST FOUR WEEKS for everyone in the house over 18
• List from pharmacy of medications you paid for LAST FOUR WEEKS ONLY (out of pocket)
• Proof of any payment for fuel oil or propane during the LAST FOUR WEEKS ONLY
• Proof of Childcare expenses you have paid in the LAST FOUR WEEKS get from daycare provider
• MOST CURRENT month's electric bill
• MOST RECENT Savings & Checking account statements, Retirement account savings
• Proof of any car repair receipts PAID IN LAST FOUR WEEKS 
• Social Security benefits notice
• Rental income statement
• Worker's Compensation payment notice
• Unemployment Compensation payment notice
• Tax Refund amount and date received
• State of child support payments received or paid
• Statement from room-mate(s) regarding division of expenses
